Periodontal staging & grading (2017)
Classify periodontitis by stage (severity) and grade (progression).
Screen: Basic Periodontal Examination (BPE) — codes 0–4 per sextant (* = furcation). Stage (by worst site interdental bone loss / CAL): I < 15% (CAL 1–2 mm) · II 15–33% (3–4 mm) · III > 33% to mid-root, ≤ 4 teeth lost · IV > 33%, ≥ 5 teeth lost or complex rehabilitation needed. Grade (bone loss % ÷ age): A < 0.5 · B 0.5–1.0 · C > 1.0; modify for smoking and diabetes. Record extent: localised (< 30% teeth) or generalised; molar-incisor pattern. Treatment follows the EFP S3 step-wise approach (see Periodontal scaling procedure).
